DA FORM 4856 and Continuation of Counseling Form

If you are talking about the DA Form 4856 counseling form there is a continuation of counseling form you can use. It can be found in any of the Counsel Quick products or in the book called the Mentor.

If you are talking about the DA FORM 2166-9-1 used for the NCOER then you can use the memorandum format or use a DA Form 4856 with a continuation of counseling form.
