Yes, the DA FORM 4856 should be used to counsel Officers.  If additional room is required you can use a continuation of counseling form or simply annotate in the key points of discussion box: see attached memorandum.

If you the counseling session is an evaluation oriented counseling you should use the evaluation forms and support forms as the basis for the conversation and use the DA Form 4856 with continuation of counseling to elaborate on specific areas.
